Community-Based Supports allow family members who are already caring for their loved ones to receive financial compensation for their dedication and time. 

Alternatively, individuals have the freedom to hire someone they trust, ensuring a seamless and comfortable care experience.

Prestige Support Services aims to improve housing stability and prevent homelessness,

to enhance health and well-being through coordinated medical, behavioral, and social supports, and support individuals in living independently within their communities. 

Housing Support Services offered through Medicaid are designed to assist eligible individuals in securing and maintaining stable, community-based housing while addressing their ongoing health and support needs. These services are not intended to pay for rent or room and board directly. Instead, they focus on providing the critical supports necessary to help individuals obtain, transition to, and sustain housing in the least restrictive and most integrated setting appropriate to their needs.

Prestige Support Services provides the following:

1. Housing Transition Services
   • Assisting individuals in identifying housing preferences and needs.
   • Supporting housing search and application processes.
   • Coordinating with landlords and housing authorities.
   • Assisting with lease negotiation and understanding tenant rights and responsibilities.

2. Housing Tenancy Sustaining Services

• Providing ongoing support to ensure successful tenancy and prevent eviction.
• Coaching on household management skills, including budgeting, safety, and cleanliness.
• Serving as a liaison between the tenant and property management when needed.
• Intervening early to resolve issues that may jeopardize housing stability.
